Prostate cancer.
Although there are a number of excellent tests to indicate the possible presence of prostate cancer, such as the digital rectal examination and the prostate specific antigen (PSA) blood test, the only certain way to diagnose the presence of prostate cancer is to carry out a prostate biopsy.
A biopsy will normally be conducted by a urologist and can be done as an 'office' procedure. There are various different ways in which a biopsy can be taken but it is common to start by taking a transrectal ultrasound. Here an instrument is inserted through the rectum and sound waves are used to produce a computer image of the prostate gland. With this image to hand the doctor can then use a hollow needle to take several small samples of tissue from the prostate for examination under the microscope. Most patients would say that the biopsy is not an especially painful procedure, but you will probably feel a stinging sensation.
The areas from which samples are taken, and the number of samples removed for testing, will depend upon the reason for the biopsy. For example, if the biopsy is simply in response to rising PSA levels then half a dozen or more samples may be taken from different areas of the prostate. However, if the biopsy is being done to examine a particular abnormal area of the prostate then only two or three samples may be collected.
Once the samples have been collected they will be examined by a pathologist. Normal prostate cells are typically of a standard size and neatly arranged in a recognizable pattern. However, prostate cancer cells normally vary in size and are irregular in shape.
If the pathologist determines that cancer is present then he will go on to grade the cancer to indicate whether it is a low grade cancer which is likely to be slow growing, or a high grade cancer which may well be aggressive and spread quickly.
A Gleason score of 2 to 4 is considered to be low and indicates a cancer which, depending on the age of the patient, may now pose a significant threat during the patient's lifetime. A score of between 5 and 7 is an intermediate grade for which treatment can often halt the disease in its tracks. Once a Gleason score reaches 8 the cancer is said to be aggressive and is likely to spread outside of the prostate gland, if indeed it has not already done so.
Testing for the possible presence of prostate cancer is recommended for all men over the age of 50 and for those men over the age of 40 who fall into a high risk category. It is important however that once prostate cancer is suspected a biopsy is carried out to confirm its presence so that treatment can be given.
Although prostate cancer is considered a disease of the elderly man, it has been known to inflict its damage on men in their early 30s. Heredity seems to play a factor especially in younger patients.
If you're a man over 50, you should consider a yearly Prostate cancer PSA (Prostate Specific Antigen) test. It's a simple a blood test that can help doctors catch the very early cases of prostate cancer. The prostate specific antigen test is like any other simple blood test. Blood is drawn by a phlebotomist and sent to the lab for analysis. A normal result from a PSA test, doesn't guarantee that your free from cancer, nor does a higher than normal result mean you DO have cancer
A PSA Count of between 0 - 3.9 nanograms per milliliter of blood is generally considered normal. Levels higher than a 3.9 ng count may indicate there may be a prostate problem which could be anything from a swelling or inflammation, infection or cancer.
It's critical that you follow your doctor's lead in regard to dealing with the Prostate Specific Antigen test results. The results are an indication that something may be happening only. To complicate matters even more, PSA levels can also increase with age. Oh and the relative size of your prostate also plays a factor in interpreting the results.
The point of this is the strongly encourage you to have an annual prostate exam and PSA blood test.
Please do not assume a high PSA reading means that you have prostate cancer. All a high PSI level reading indicates is that there "may" be a problem. Two out of three people with elevated the PSA readings DO NOT have a malignancy. So don't panic!
Here's a few very good reasons why you should consider getting an annual PSA blood test
a. Your doctor is much better able to detect potential prostate cancer even though you have no symptoms.

Note that a Harvard study done in 2005 indicated that those men having a yearly PSA test and monitoring of PSA levels were almost 3 times less likely to die for prostate cancer than those who didn't bother with the PSA
Finally, according to the Centers for Disease Control ( CDC ), is the introduction 20 years ago of any use of the PSA test, deaths from prostate cancer have fallen by one third. Prostate cancer if caught in its early stages is treatable and curable. Don't wait until it is too late to have this important test done.

Benign prostate hyperplasia begins through ...
...the formation of small nodules in the periurethal glands in the inner layers of the organ. This is known as hyperplasia. AT the same time there is also an enlargement of the glandular cells which is called hypertrophy. It can take a long period of time for these changes to slowly occur.
As the prostate tissue expands it will eventually begin to tighten around the urethra which will cause a partial or complete obstruction of urine flow from the bladder. When this occurs the detrusor muscles will hypertrophy to compensate for this increased resistance, somewhat mitigating the symptoms. Unfortunately this will cause decreased bladder compliance and bladder instability. It is at this time that the signs and symptoms of BPH begin to appear.
This includes a weak urinary stream, increased time to urinate, hesitancy, incomplete emptying of the bladder, and post urination dribbling. Irritation of the bladder and urethra can also occur which can lead to an increased urinating frequency, urgency, incontinence, nocturia, dysuria, and bladder pain.
If an enlarged prostate is not treated and reduced in size a multitude of complications can begin to develop. The bladder can stretch and distend as more and more urine begins to be retained inside it. This can result in out-pouchings of the bladder wall, often referred to as Diverticula. Any distension can cause a blockage of the ureters, causing a back up of urine and infection back into the kidneys, causing kidney infections.

In addition, medicines which act on the 'Mansa' dhatu (tissue) are useful in this condition. These medicines include Triphala (Three fruits), Musta (Cyperus rotundus), Patol (Tricosanthe dioica), Nimba (Azadirachta indica), Draksha (Vitis vinifera) and Kutaj (Holarrhina antidysentrica).
Patients who do not respond to the above mentioned medicines are given additional medicines like Kapikacchu (Mucuna pruriens), Kuchla (Strychnos nuxvomica), Samudrashosh (Argyreia speciosa), Ashwagandha (Withania somnifera), Vacha (Acorus calamus), Varun (Crataeva nurvala), Yav-Kshar, Apamarg-Kshar, Mulak-Kshar and Trivang-Bhasma.
Treatment needs to be tailor-made for each patient, since not all patients respond to the same medicines. Depending upon the presenting symptoms and perceived enlargement of the prostate gland, a combination of 2-3 medicines is used for a minimum period of 2 weeks.
